I understand your requirement as follows.
1.You want to hide Second title-it can be done using screen modifications(t588m),here you have an option to hide.
2.You would like to have 'Father Name' as the additional field and you are maintaining father name in Family Member dependents (IT0021).May I know any specific purpose for capturing this field here in this infotype? If your requirement satisfies by capturing through IT0021,you can convey the same to your client,otherwise you need to talk ABAP Consultant to explore further as Father Name fields is not available in this table(t588m)
